Most patients have their records scattered across several hospitals, portals, and labs. I gather everything, including chart notes, pathology, labs, imaging, and medications, into one organized, chronological binder with a two-page summary on top. When you see a new specialist, you hand them the summary instead of trying to recall years of history in a short appointment.

Appointment Preparation and Accompaniment

Before a major appointment, I prepare a focused list of questions with you. I attend in person or by video as your note-taker and second set of ears, then write a clear summary afterward.

Family Briefing Memos

After each appointment, I send out-of-town or working family members a plain-language update so everyone knows where things stand.
